Declaration

Our participating companies have agreed to abide by the ombudsman's decision. This means that if we propose a remedy that you're happy with, the firm will be required to put this in place. We will make sure that this happens.

There are some types of complaint that we cannot consider. If this is the case we will let you know as soon as possible. Please check our website for common examples of complaints that we can and cannot handle.

Statement

I would like the ombudsman to consider my complaint. I confirm that the information I have given is true and accurate, to the best of my knowledge.

I understand that:

  • on receipt of this complaint the information I have provided will be made available to the company that the complaint is about
  • the ombudsman may need to exchange information about my complaint with the company I have complained about and with other relevant organisations
  • the ombudsman handles complaints differently from the courts - usually settling disputes by phoning and writing to the two sides, not by holding hearings in person
  • the ombudsman may publish examples to show where things can go wrong based on real cases. When doing so the ombudsman will always respect privacy and keep personal information confidential
  • I understand that Ombudsman Services will only use my information to process my complaint or to improve service quality
